Postpartum constipation and hemorrhoids: the recovery nobody mentions
Of all the postpartum topics that get skipped in the discharge talk, this one generates the most quiet 3am searching. It's nearly universal, it's temporary, and the fixes are unheroic and effective.

Why it happens to almost everyone
The deck is stacked: progesterone slowed your gut for months, delivery pushed on everything, opioid pain relief and iron supplements both constipate, healing makes you guard, and fear of the first bowel movement makes you delay — which makes stool harder, which justifies the fear. Hemorrhoids — swollen veins at the rectum — ride the same wave: studies put perianal problems (hemorrhoids and fissures) at around 40% of women in pregnancy and postpartum, with the final weeks and delivery itself the peak drivers.
The toolkit
Water first and most — nursing raises your fluid needs on top of recovery. Fiber with intent: fruit, vegetables, oats, prunes. Stool softeners are routinely recommended postpartum and compatible with breastfeeding — start early rather than after trouble sets in. Move as you're able; even hallway walking helps the gut. Go when the urge comes, feet on a small stool to improve the angle, and don't strain — support a tear repair with a clean pad of tissue if that helps your confidence.
For hemorrhoids: witch-hazel pads (chilled ones do double duty), sitz baths a couple of times a day, short courses of over-the-counter hydrocortisone or hemorrhoid cream, and side-lying instead of long sitting. Most delivery-related hemorrhoids shrink substantially within weeks.
When it's a call, not a lifestyle fix
Four to five days without a bowel movement, severe abdominal or rectal pain, more than streaky bleeding, or a hemorrhoid that turns abruptly hard and severely painful — a thrombosed hemorrhoid, which an OB can treat quickly, especially early — all warrant a call. So does any new difficulty controlling gas or stool: that's not a constipation problem, it's a pelvic-floor or repair question with real answers, and the only wrong move is not mentioning it.

- Are stool softeners safe while breastfeeding?
- The standard ones (docusate) are considered compatible with nursing and are routinely prescribed postpartum. Stimulant laxatives are a when-needed, ask-your-OB tier rather than a daily default.
- Will the hemorrhoids go away completely?
- Most that arose from pregnancy and delivery shrink markedly within a few weeks. Ones that persist, bleed repeatedly, or keep flaring are worth an appointment — office treatments exist well short of surgery.
- How long does postpartum constipation last?
- Usually the first week or two are the hard part — softeners, fluids, and mobility carry you through the worst of it. If it persists past a few weeks, mention it at a visit; iron dosing or other adjustments may help.
